aria-labelledby=\"elementor-tab-title-1961\"><ol><li>Applicants shall submit the following:<ol><li>Written application (RTA)<\/li><li>Plans, schematic diagrams<\/li><li>Company\u2019s business permit, DTI or SEC registration<\/li><li>Secretary\u2019s Certificate of Authorized signatory<\/li><li>Photocopy of one (1) government issued ID of the authorized signatory<\/li><\/ol><\/li><li>The Corplan will notify the TSD for the evaluation of the applicant and inspection of the site applied for. TSD shall inform the applicants of the date of inspection. Expenses for inspection shall be at applicant\u2019s cost.<\/li><li>TSD shall prepare written evaluation, inspection report and summary of allowable attachments and poles posts signed by TSD staff and applicant\u2019s representative and forward the same to Corplan.<\/li><li>The Corplan shall prepare Pole Lease Agreement\/Contract.<\/li><li>Corplan will inform FSD for the preparation of Statement of Account.<\/li><li>Applicant shall advance annual pay rental fee<\/li><li>Corplan shall facilitate notarization of Pole Lease Contract<\/li><li>Corplan shall issue Notice to Attach to the applicant, copy furnished the TSD\/Area Office.<\/li><li>The attachment to the poles shall be made only in the presence of the TSD\/Area Office representative, as such, Applicant shall facilitate the presence of TSD\/Area Office during attachment. Only those included in the summary of allowable attachments and poles shall be allowed.<\/li><li>TSD\/Area Office will notify Corplan once the attachment has been completed.<\/li><li>Corplan shall forward copy of the Pole Lease Agreement\/Contract to FSD for monitoring of payment, and to IA for monitoring of compliance with the provisions of contract.<\/li><\/ol><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<div class=\"elementor-toggle-item\">\n\t\t\t\t\t<div id=\"elementor-tab-title-1962\" class=\"elementor-tab-title\" data-tab=\"2\" role=\"button\" aria-controls=\"elementor-tab-content-1962\" aria-expanded=\"false\">\n\t\t\t\t\t\t\t\t\t\t\t\t<a class=\"elementor-toggle-title\" tabindex=\"0\">REQUEST FOR ADDITIONAL ATTACHMENTS<\/a>\n\t\t\t\t\t<\/div>\n\n\t\t\t\t\t<div id=\"elementor-tab-content-1962\" class=\"elementor-tab-content elementor-clearfix\" data-tab=\"2\" role=\"region\" aria-labelledby=\"elementor-tab-title-1962\"><ol><li>Applicants should have no previous violations or any unsettled payables on the existing contract<\/li><li>The Corplan will notify the TSD for the evaluation and\/or inspection of the request for additional attachment. TSD shall inform the applicants of the date of inspection. Expenses for inspection shall be at applicant\u2019s cost<\/li><li>TSD shall prepare written evaluation and inspection report and forward the same to Corplan.<\/li><li>Corplan will inform FSD for the preparation of Statement of Account<\/li><li>Applicants shall pay rental fee. The duration of the period for additional attachment shall not exceed the original\/existing Pole Lease Agreement\/Contract.<\/li><li>Corplan shall issue Notice to Attach to the applicant, copy furnished the TSD.<\/li><li>To ensure proper attachment and observance of height, attachment to the poles shall be made only in the presence of TSD\/Area Office representative, as such, \u00a0Applicant shall facilitate the presence of TSD\/Area Office during attachments. \u00a0Only those included in the summary of allowable attachments and poles shall be allowed.<\/li><li>TSD will notify Corplan once the attachment has been completed.<\/li><li>Corplan shall forward copies of the request for attachment, SOA and Notice to Attach to FSD for monitoring of payment, and to IA for monitoring of compliance with the provisions of Contract.<\/li><\/ol><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<div class=\"elementor-toggle-item\">\n\t\t\t\t\t<div id=\"elementor-tab-title-1963\" class=\"elementor-tab-title\" data-tab=\"3\" role=\"button\" aria-controls=\"elementor-tab-content-1963\" aria-expanded=\"false\">\n\t\t\t\t\t\t\t\t\t\t\t\t<a class=\"elementor-toggle-title\" tabindex=\"0\">RENEWAL<\/a>\n\t\t\t\t\t<\/div>\n\n\t\t\t\t\t<div id=\"elementor-tab-content-1963\" class=\"elementor-tab-content elementor-clearfix\" data-tab=\"3\" role=\"region\" aria-labelledby=\"elementor-tab-title-1963\"><ol><li>Applicants shall submit the following:<ol><li>letter for application for renewal<\/li><li>map and location of the existing attachment<\/li><li>Company\u2019s business permit, DTI or SEC registration (if none were previously submitted)<\/li><\/ol><\/li><li>The Corplan will coordinate with TSD, IA and the applicant for the inventory and inspection of the existing attachment. TSD shall inform the applicants of the date of the inventory inspection.
